The incident occurred when the pick-up was caught on video aggressively blocking a passenger bus along a public highway, repeatedly preventing the bus from overtaking in what LTO chief Assistant Secretary Vigor D. Mendoza II described as 'a grave display of abuse and lack of discipline that endangered other road users.' The video circulated widely on social media in June 2025, prompting swift action from authorities.

LTO issued a show cause order requiring the registered owner, a resident of Bato, Camarines Sur, to appear before the LTO Intelligence and Investigation Division in Quezon City on June 20, 2025. The owner was ordered to submit a written explanation regarding why they should not be held liable for reckless driving under Section 48 of Republic Act No. 4136, and why their driver's license should not be suspended or revoked for being an 'improper person to operate a motor vehicle.'

The vehicle was placed under alarm status, preventing any transactions involving the pick-up while the investigation was ongoing. Authorities were also verifying whether the registered owner was actually driving the vehicle at the time of the incident. The action was taken in line with President Ferdinand Marcos Jr.'s directive to hold accountable individuals who compromise public road safety.
